just:
1. Create database
2. create database user and password
3. Grant full permission to user
4. Setup your php script. 

Various script may have different way to setup the script.
At the script config, server you should put "localhost" put your full database name, normally "yourcpanelusername_yourdatabasename" same as you database user.</description>
